Comprehensive medical care delivered in the comfort of your home
Complete primary care delivered in your home. Annual wellness visits, preventive care, and ongoing management of your health, all without leaving the house.
Ongoing management of conditions like diabetes, heart failure, COPD, hypertension, and kidney disease. Regular monitoring, medication adjustments, and coordination with specialists to keep conditions controlled and prevent complications.
The days after a hospital discharge are high-risk. We provide prompt follow-up visits to review discharge instructions, reconcile medications, and catch problems before they lead to readmission.
We review all your medications, including over-the-counter drugs and supplements, to check for interactions, simplify where possible, and make sure everything is working together safely.
Specialized care for patients with Alzheimer's disease and other forms of dementia. We work with families to manage symptoms, plan for the future, and maintain quality of life.
For patients dealing with general decline, weakness, or multiple age-related conditions, we focus on maintaining function, preventing falls, and supporting independence as long as possible.
Palliative care focuses on comfort and quality of life for patients with serious illness, whether or not they're pursuing curative treatment. It's an extra layer of support alongside regular care, not a substitute for it.
We communicate with your specialists, order labs and imaging, coordinate with home health agencies, and make sure everyone involved in your care is on the same page.
Unhurried conversations about your goals, values, and wishes for future care. We help patients and families work through difficult decisions with clarity and compassion, so that the care you receive reflects what matters most to you.
Dr. Matulis comes to you, whether that's your own house, an assisted living facility, or a family member's home. Visits typically run 45 to 60 minutes. He brings the equipment for a thorough exam, orders labs and imaging, adjusts medications, and stays in contact with any specialists involved in your care.
You'll see the same doctor every time. No rotating providers. No starting over.
Caring for an aging parent—especially one who can't easily leave home—can be exhausting.
We're here to help carry that weight.
Family members are welcome to join home visits. Some of the best conversations happen that way, when everyone is in the same room asking questions and making plans together.
You know the patients who are going to bounce back. The ones without reliable primary care. The ones who can't get to follow-up appointments. The ones whose home situation is a black box.
We can help.
