We review all your medications—including over-the-counter drugs and supplements—to check for interactions, eliminate unnecessary medications, and ensure everything is working together safely.

Dementia & Cognitive Care

What to Expect

Dr. Matulis will come to your home—whether that's your own house, an assisted living facility, or a family member's home. Visits typically last 45-60 minutes. He'll bring the equipment needed for a thorough exam, can order labs and imaging, adjust medications, and coordinate with any specialists you see.

We work alongside home health agencies and skilled nursing facilities—not in place of them. When your patients need a primary care physician who can actually see them at home, we're here.

How We Work Together

Medicare & Eligibility

Who Qualifies

Our practice is designed for patients who meet these criteria:

  • Traditional Medicare (Original Medicare): You have a red, white, and blue Medicare card—not a Medicare Advantage plan. If you're not sure which type you have, just give us a call and we'll help you figure it out.
  • Homebound or have difficulty leaving home: Getting to a doctor's office requires considerable effort, special transportation, or assistance.
  • Live in our service area: The Oxford Hills region, including Auburn, Lewiston, Turner, and surrounding communities.

What Medicare Covers

Medicare covers medically necessary home visits for patients who have difficulty leaving their home. This includes:

  • Home visits from a physician
  • Annual wellness visits
  • Chronic care management
  • Care coordination
  • Lab work and imaging (ordered and performed at approved facilities)

What You Pay

For most patients with Traditional Medicare and a supplemental (Medigap) policy, there is little to no out-of-pocket cost for home visits.

If you have Traditional Medicare without a supplement, you may be responsible for the standard 20% coinsurance after your deductible is met.

We don't charge concierge fees. This is not a concierge practice. We bill Medicare directly for our services.

What to Expect on Your First Visit

Before the Visit

  • Gather all your medications—prescription, over-the-counter, and supplements
  • Have your Medicare card and any insurance cards ready
  • Write down any questions or concerns you want to discuss
  • If family members want to be present, let us know when scheduling

During the Visit

The first visit typically takes 45-60 minutes. Dr. Matulis will:

  • Review your complete medical history
  • Perform a physical examination
  • Review all your medications
  • Discuss your health goals and concerns
  • Assess your home environment for safety
  • Create an initial care plan together

After the Visit

  • We'll schedule follow-up visits based on your needs
  • If labs or imaging are needed, we'll coordinate how to get them done
  • Family members will receive updates (with your permission)
  • You'll have a number to call or text with questions
