For Families

Getting started is simple:

1. Call or text us at 207-492-4642. Tell us about your loved one and their situation.
2. We verify Medicare eligibility. We'll confirm that your loved one has Medicare and meets the criteria for home-based care.
3. We schedule the first home visit. Dr. Matulis will come to the home, meet the patient and family, review medical history, and create a care plan.
4. Ongoing care begins. Regular visits, available by phone, and coordination with other providers.

Referring a patient is easy:

Please include: patient name and contact info, Medicare ID, diagnosis and reason for referral, recent discharge summary (if applicable), and current medication list.
